@tzucuskie the apical margin of the mucosa is not tacked down separately; it just heals by secondary intention to the apical margin of the graft. I use a Global scope with a beam splitter to a Sony camcorder. I record on DV tapes and keep a library of tapes/pts. Each pt has their own tape so I can record post ops and other follow ups.

Great suture technique, thanks for passing that on. At the recipient site the apical mucosal margin is not sutured and allowed to passively adapt to the superior edge of the sutured graft or do you tack it down also? Could you please share the video camera brand/setup you are using also? The quality is great and I would like to do some video in my office to share with my patients. Thanks!

The graft was thinned to a uniform thickness with the scalpel and what I was removing was fat from the palate. AlloDerm is an alternative graft material that MUST be COVERED and can be used as an alternative to CT grafting from the palate. In certain instances CTG is still preferred.

AlloDerm does not increase the keratinized gingiva. This is a palatal gingival graft (not CT), which is noted in the harvest of the donor tissue. CT is from under a flap on the palate. See CTG videos on periEau channel. With such floppy mucosa, gingival grafting gives you bound down keratinized gingiva.

Thanks so much for the kind words!! I never drink caffeinated coffee when I work, I try to get plenty of sleep and I am an exercise addict! The microscope really puts me into a "focused" world and having an awesome assistant makes all the difference as well.

No endo. Reduction was not very deep. Gerestore does not seem to irritate the pulp. We got attachment to the Gerestore in this case but if not we just have to maintain a deep sulcus. Any attachment is probably long junctional epithelial attachment.

great job dr.kwan want to ask u that tooth was not endodontically treated right? u did deep reduction into root n put that flowable copmosite i thought is it irritative to pulp with out liner or base due to high level of monemer in it? then how about pocket depth it will be reduced? and the attachment between connective tissue in the base of reflected flap and cured composite how it well be done? thank u for this great job

I'm a dental nurse, and I love anything with pus or blood, anything gory! .... But perio..... I just can't handle it. Teeth are not suppose to be mobile!!! Creeps me out.... *shudder* I could never be a hygienist.... Bleeding gums require a dentist and interdental cleaning. To have good teeth it's real simple, brush your teeth with a fluoride toothpaste twice a day, clean between the teeth, chew sugar free gum and limit sugar consumption. And pay the dentist a visit at least yearly!

Well, if your gums are bleeding it coulde be that you have gingivitis, but don't worry too much because it is reversible. If you can, brush your teeth 2 times a day and floss and if you don't have access to a tooth brush, chew a sugarless gum for about 2 minutes and rinse with water. If you don't have insurance or if you do, try to go to the dentist and get a prophylaxis(cleaning) and go for another visit in six months.
